Getting to Know Each Other
As you or your child become a new patient at Dr. Miller's office, we strive to give you all of the information you desire. We encourage you to ask questions and read the materials we provide. One of our top priorities is to create an open and friendly atmosphere for our patients and their families. We want to learn about you, too. As a patient you will be like a part of our family. Therefore the first thing we ask you to do is complete the Medical/Dental History form, which will be mailed to you with your first appointment letter. A downloadable version of this form is available on this website (Medical/Dental History Form). It is important for us to know about the health history of every patient, so our Treatment Plan will support any dental or medical work that has been done, is underway, or is planned to be done. Beyond discussing your medical information, we are delighted to hear about activities and endeavors that our patients enjoy in their spare time. We support many local organizations and events, so chances are you will see our smiling faces out in the community as well as in the orthodontic office!
The Initial Examination (Your First Appointment)
Every patient begins with an initial orthodontic examination, where Dr. Miller evaluates and notes clinically evident issues. She will discuss these issues and treatment timing with the parent and patient or the adult patient. Some patients may not be ready for treatment. If this is the case, we will observe you periodically to determine when treatment should begin. If it is determined that orthodontic intervention is indicated at the present time, orthodontic diagnostic records will be recommended. Diagnostic records are composed of plaster study models (duplicates of your teeth and your bite), digital orthodontic x-rays and digital facial and intra-oral photos. Diagnostic records enable us to identify specific conditions and determine the best course (or courses) of action to deal with those issues. It is important to realize that no one can determine the best treatment plan, the exact length of treatment, and the total treatment cost with out diagnostic records. Better yet, we do all of our diagnostic records in the office (you are not sent away to a third party) and we can do them the day of your initial orthodontic examination! Our Initial Examination appointment is performed at no charge to the patient. If the patient would like to go forward with diagnostic records, a fee of $325 is due at the time of service.
The Consultation (Your Second Appointment)
Each parent or adult patient will receive a written proposal outlining orthodontic issues and treatment recommendations. Every patient has an individualized treatment plan designed to achieve a healthy, beautiful, and confident smile! The treatment plan will be discussed and explained thoroughly with regard to treatment modality, cost, and time. Dr. Miller will use the components of your diagnostic records and computerized visual aides to explain and demonstrate. Dr. Miller brings an extraordinary level of understanding, confidence, experience and communication skills to these discussions. We encourage you to ask questions and express feelings during this process—we are here to be of service to you as you gather information.
Our financial coordinator will meet with you at the consultation appointment to discuss your orthodontic fee, financing options, insurance coverage, and length of payment plans. Our most popular payment plan is one that consists of a 25% down payment and equal monthly payments of the remainder over the course of treatment. Please see the Financial Information page of this website for details.
